Emotional Support Animal Letter (ESA)

All fees are out-of-network. Insurance superbills can be provided for clients to submit for reimbursement upon request. Clients should check with their insurance plan to see reimbursement rates. All payments are confidential through Square Payment Processing (Visa, MasterCard, American Express, Discover).

You are entitled to a “Good Faith Estimate” outlining the price of your future medical and mental health care. Healthcare providers are required by law to provide uninsured or underinsured patients with an estimate of the anticipated costs for medical services, including psychotherapy services. The total expected cost of any non-emergency medical services, including psychotherapy services, must be provided to you in a good faith estimate. Before scheduling a service, you can request a Good Faith Estimate from any provider, including your health care provider. You have the right to challenge a bill if it is at least $400 more than your good faith estimate. You can visit www.cms.gov/nosurprises if you have any questions or would like more information about your right to a good faith estimate.
